Health Maintenance of Musculoskeletal System during long distance travel

Eighty percent of population suffered from more than one episode of severe back pain, also called lumbago during their lifetime. He or she have to rest, absent from the job, even sometimes was forced to leave their job. Even more people annoyed by neck and shoulder pain, which may refer to scapular area, upper arm and even to the hands. All these pain and soreness are related to the disorder of spine, which is unique to human being after our ancestor started to stand on their feet. Many of these problems need long term medication and physical therapy, some even need to resorted to surgery. However, the surgery may not resolve the problem forever, many relapse and need further surgery, sometimes some sequels developed and the patient may never stand again. The prevention of back and neck pain relied on three pillar which we called 3 P of spine health. Position, Prediction and Protection

The first P is Position (also called posture), which is the most important of all. Position of the spine during sitting and standing decided the modes of force transmission of the upper body (half of the body weight) to the lower extremities. When the body is upright, there is natural curvature like a double S at the side view. The neck and lower back were curved forward, the chest part also called thoracic spine curved backward. All these are built with reason. The chest curved back together with the rib cage form a barrel shape space where lung lying inside works very efficiently like a bellow to sustain our life by drawing fresh air in and push carbon dioxide out. To balance the backward curve and keeping the head upright, the neck and lower back have to reversely curve in compensation.

Like a tower building, vertical tower is much easier to maintain upright than a tilting tower, because the gravity force is always there to bring down it. To keep body upright, we need the trunk muscles and inter-vertebral ligaments works together all the time to bring the 24 vertebral bodies (like blocks of stone put one on another in stone column) upright as long as we keep our body standing, walking, working or sitting but lying. Modern principle of back school such as Pirates emphasis the training of deep core muscles of back to keep the body upright, avoid loss of natural curvature of back, which put strain on the ligaments connecting the vertebral bodies. Chronic strain of ligament produces micro-injury to the ligament which invokes inflammation and pain. Severe acute injury of ligament such a sudden sprain may rupture the fiber of ligament which contains the disc. The disc under pressure may extruded from the inter-vertebral space into the spinal canal and impinging a nerve root, and a referred pain to the arm or leg developed which may need a surgery (called discectomy ) to remove it.

So the first thing to prevent back pain or herniation of disc is Position of spine, the posture.

The second P is Prediction. After learning the mechanism of back injury, people has to learn which posture is dangerous to the back and neck. This is also the main function of back school, Health Education? Most includes Dos and Don�ts. The best policy is practice good postures and keeps away from bad into an instinct reaction without thinking.

The Third P is Protection. For those who suffered from repeat back pain, some kinds of protection using brace or support is necessary. Especially when some structural change such as kyphosis ( revere curvature of neck or lower back spine column ) developed. Effective protection can calm the inflammation and pain of the spine. But many protection devices are not effective, that is why custom making is common in the business.

During Long distance travel, such as air traveling, taking coach or train, people used to take a nap to recover from fatigue of traveling. Without proper protection, people may get injury of neck or back. Because when we fall into deep sleep, in the stage of quick eye movement stage, all the muscles(the engines and battery of the body motion) of the body are paralyzed to get rest and recharging. That is why our neck sways while we sit and nap. Without enough support, we may get injury and acute pain from over-stretch of ligaments of neck and back and displacement of the disc tissue within the disc space. In the neck, people called it Try neck? and at the back Back Strain? To get the neck and back supported is very important during long distance travel.

Most of the seats in public transportation are without design for protection of the neck and back. Some people bring along with them a medical grade neck collar, a back support designed for car seat or a cushion from home. However, these are bulky and not easy to carry.
